Is your dog currently fit and well, with no signs of vomiting, diarrhoea, coughing, contagious illness, weeping eyes, open wounds or stitches?
Does your dog have any medical conditions, allergies, injuries or mobility needs?
Does your dog take medication? If yes, please explain reasoning for medication.
Is your dog brachycephalic (flat-faced), or do they have any history of noisy/difficult breathing, overheating, exercise intolerance, airway surgery or collapse? If yes, please explain.
Does your dog have any dietary needs, food allergies or treats they must avoid?
How does your dog generally respond when they are on leash and they see unfamiliar dogs?
How does your dog generally respond when they are OFF leash and they see unfamiliar dogs?
Does your dog have any experience in group daycare, dog walking groups, boarding or social clubs? What went well, and what did not?
Has your dog ever snapped, growled, lunged, barked excessively at, chased, injured, or been injured by another dog? Please explain honestly, this can often be normal, appropriate behaviour .

If your dog becomes injured or unwell while attending daycare we will attempt to contact you immediately. If we can not get a hold of you, we will contact your emergency contact. If we believe your dog needs emergency veterinary care, we will bring them to our local veterinary clinic. If your dog needs emergency treatment, and we can not reach you for authorisation, what limit would you like to pre-approve for emergency treatment (many owners select their dogs insurance policy limit)?
